Part 1 - The Adult Disability Report

The first step in applying for SSI is to fill out the Adult Disability Report.

Finding the form and instructions

You can find the form online during these hours:

  • Monday - Friday 4:00am to 12:00am (Central Time)

  • Saturday 4:00am to 10:00pm

  • Sunday 7:00am to 10:30pm

Filling out the Disability Report

  • You can fill it out and submit it online or you can fill it out on your own and bring it in when you do your interview.

  • You will need to sign a form to give Social Security permission to get medical information from your doctors.
