Create A Transition Plan

The transition plan is a detailed plan for helping your child meet their goals for after high school.

It is a document that outlines the services and supports for your child to help them meet these goals. The IEP team will create this with you.

The school is legally required to create one for all students on IEPs who are 16 and over.

Here's what you can do:

  • Work together with the IEP team to write the  Transition Plan.

  • Involve your child in this process if they are able. 

  • Be sure to ask about services, special programs and Accommodations.
