First, talk as a family and think about your child's future goals, and what kind of support they'll need to reach them:
Talk to your child about this, if you can.
Encourage them to think about their interests, values, and future goals.
Give them opportunities to practice making decisions and to learn about themselves.
Get advice from friends, teachers, or service providers.
These conversations will help the IEP team (including you!) create a transition plan that is really what your child wants.
