At the second home visit, the Support Coordinator will work with you to make a POC, based on information you gave at their first visit.
The Support Coordinator will help your family member choose providers from the Freedom of Choice listing.
(You can see it toward the bottom of this screen.)
The POC should have answers to these things:

What kinds of services your family member will get, and how many services they will need (This includes waiver services and all other services.)

Who will provide the services

Speak up if you don't get what you need! You know your child best, and you have a right to push for the services that support they goals.
