Making the Plan of Care (POC)

At the second home visit, the Support Coordinator will work with you to make a POC, based on information you gave at their first visit.

The Support Coordinator will help your family member choose providers from the Freedom of Choice listing.

(You can see it toward the bottom of this screen.)

The POC should have answers to these things: 

  • What kinds of services your family member will get, and how many services they will need (This includes waiver services and all other services.)

  • Who will provide the services

  • /9*What you can do if you have questions or problems.

  3. Choose which services you're interested in and click on "Browse Providers"

  4. Scroll down to see a list of qualified providers to choose from
