What to do when your name comes up

When they have waiver services available for you, a company called Statistical Research, Inc. (SRI) will contact you.

You may hear people refer to this as the time when your protected date is served. Basically, it means that your name has come up on the Registry and it's your turn!

Here's what you can do: 

  • Contact Statistical Research to accept the Waiver. (We'll tell you how on the next screen.)

  • Start the process of connecting with a Support Coordinator and setting up your plan of care. A Support Coordinator is like a case manager and will help you through the process.

  • To learn more, see the section in the menu on left about Setting up a Plan of Care (POC).

Of course, you have a right to not accept the Waiver. But this is not a good idea unless you have learned about what they offer, and you're absolutely sure your family member does not need any of the services.
