Agree to the placement
If you agree with the proposed placement, the next step is to sign the placement form. If you don't agree or want to schedule another meeting to talk about it, you don't have to sign!
Again--you can take this home and think about it before you sign anything! Remember, you - the parent - are a member of the IEP Team. You and the rest of the Team should determine your child's placement together.
What you can do:
- At the bottom of theTeam Determination of Educational Placementform,check the box:I consent to the placement decision. (See the image below.)
- Sign and date the form.Note: If your child is 18 or older, they will need to sign the form, unless you have legal permission. See theExceptional Lives Guardianship Guidefor more info on making decisions for your child.
- Make copies of the placement form. Put one inyour IEP binderso it's ready for next year's IEP meeting.
- Give the signed placement formto your child's school.You can drop it off in person or mail it.Usually you sign this form and the IEPat the same time.
