Fill out a Thoughts & Concerns Form to help with 504 planning
This is like apractice formto help you think about your child's needs. It's not an official form, but it may help you to prepare for meeting with the 504 Coordinator.
Here's what you can do:
- Jot out your answers to these questions below. Try to keep them brief and to the point - no more than 2 to 3 short paragraphs for each question.
- When you're done, click on the red Save My Completed Form button below.
- Then download the form, print it, and share it with your child's teacher and the 504 Coordinator. Or you can just use it for yourself!
Note: Not all sections may apply to your child. Don't feel you have to put something in every box!
