Applying for Healthy Louisiana (Medicaid)

Here's what you need to do

There are 2 parts to applying for Medicaid:

  1. First you have to see if your family member qualifies. They call this "determining eligibility". The first step is to fill out an application and submit it.

  2. If you qualify, then you enroll in one of the 6 health plans (MCOs).

You can apply online, by phone, by mail, or in person. If you can apply online, it might be faster.

This can be complicated! If you have questions about enrolling in a health plan, call: 1-855-229-6848

If you qualify, enroll in one of the 6 health plans:

  1. Aetna

  2. AmeriHealth Caritas Louisiana

  3. Healthy Blue

  4. Humana (new choice since 2022!)
  5. Louisiana Healthcare Connections

  6. UnitedHealthcare Community Plan

Healthy Louisiana may have to choose an initial plan for you, but you have 90 days to change to a different plan if you want.

After that, you will be able to change only during the open enrollment periods each year.

You will get 2 health insurance cards: Medical Card Icon

  • 1 for Healthy Louisiana: This shows that you are a Medicaid member.
  • 1 for your Health Plan: This is what you show at your doctor appointments.
