When we say "Transition", we're talking about how a child or young adult moves from school-based services to life after school.
The school has many ways to support students who have disabilities and help them prepare for life after high school. This process is called Transition because your child will transition to life as an adult.
The education system in Louisiana aims to prepare all students for success after high school. We'll talk about how this happens for a child with a disability between the ages of 16 and 22.
