The next step is to do the Intake or Family Assessment with the EI program.
What to expect:

You'll talk with your EI program, either on the phone or in person. Usually they will come to your house, but they can also meet you somewhere else if you prefer.

They'll tell you about EI, and get to know a bit about your child and your family.

They'll gather information to help them plan an evaluation of your child.

They'll ask you to sign a form that gives your consent for the evaluation. It's called Prior Written Notice and Consent for Evaluation/Assessment.

What you can do:

Describe your child, your family and your concerns.

Share information from doctors, specialists or teachers.

Ask any questions that you have.

Sign the form if you agree to have the evaluation, and then schedule it.
