The Clinical Team Report confirms that your family member has an intellectual disability that impedes their decision-making.
Each of them must have experience in evaluating an intellectually disabled person.
Here's what you can do:
1. Click the link at the bottom of the page to download a PDF copy of the clinical report form:
2. Arrange for each of the 3 professionals to meet separately with your family member and fill out the form.
3. Make sure they examine your family member within 180 days(6 months) of the date the Petition will be filed.
