Transition planning to prepare for life after school

It may seem very far away, but your child's future goals may affect the kinds of services they get in the next few years.



There's a lot you can do to make sure they get the most out of their last few years in school.

Students with disabilities can get services from their IEP and also connect with outside agencies to support them as they move into adult life.


For children over 14, the IEP process must include planning for their life after high school. This is called transition planning, and it

Here's what you can do:

  • Think about what you see your child doing after high school (or after they are 22 and no longer in a school program):

    • Living with you?

    • Getting a job?

    • Living in a group home?

    • College program?

  • Talk to your child (if possible).Together, think about their interests, values, and future goals.

  • Give them a chance to practice making decisions and learn about themselves. These conversations will help with the transition planning process. They may want to come to the IEP meeting to describe their goals themselves.

  • Get advice from friends, teachers or service providers.
