During the transition process, you will have many ways to talk to your child about what they want and what they can do.
It’s important for you to be very aware of your child's skills and interests, their challenges, and what kind of support works well for them.
Here’s what you can do:
Talk to your child often about school and future plans.
Monitor their grades and progress reports carefully.
Look at all the assessments they do (school assessments, informal skills assessments, etc.).
Keep in touch with their teachers, counselors and IEP team. Make sure you understand how your child is doing and what kinds of choices they have for the future.
Ask for meetings with the team any time. If your child has an IEP, you should be talking about transition at each IEP meeting. You can ask to meet at any time beyond the IEP meeting!
Write down what you know about your child's interests, abilities, and needs for support.
Be as specific as you can.
This can be helpful later when you're applying for services!
