Next steps

Once you've filled out the forms and accepted the waiver, here's what happens next:

  1. You'll choose a support coordinator and meet. 

    • You'll pick from a list of agencies.

    • This person will help you to decide what services your family member needs, and how to find them.

  2. They will help you create a Plan of Care or POC for your family member.

    • This will describe the services and who will provide them. 

  3. The coordinator will help you understand your rights and responsibilities.

  4. Your family member will start getting the services.
