Once you've filled out the forms and accepted the waiver, here's what happens next:

You'll choose a support coordinator and meet.

You'll pick from a list of agencies.

This person will help you to decide what services your family member needs, and how to find them.

They will help you create a Plan of Care or POC for your family member.
- This will describe the services and who will provide them.

The coordinator will help you understand your rights and responsibilities.

Your family member will start getting the services.
